Be prepared - Have the following information ready for your telephone application (TeleApp) interview
Employment details, including name of employer, length of employment, and your job duties
Current income and net worth
Any foreign travel you have planned for the next 12 months
Any adventurous activities you take part in, such as scuba diving, mountain climbing, airplane flying, etc.
Medical history details, including date of onset, treatment, and recovery for any medical conditions you have experienced
Names of physicians or hospitals that have provided care for you within the last 10 years
Names and dosages of medications you have been prescribed
A TeleApp interview will last 30 minutes or less. Please note that more complex medical histories or the use of an interpreter may result in a longer TeleApp interview.
After the call, you will receive a copy of the completed telephone application, along with instructions to change any of the information provided, if needed.

Completing your Medical Requirements
New York Life, like other insurance companies, may require a limited medical exam based on your age and the amount of insurance coverage requested. If an exam is needed, your agent will schedule a time for it to be completed.
YOUR EXAM MAY INCLUDE:
Measurements of blood pressure, pulse, height, and weight
A blood and/or urine sample
An electrocardiogram to measure the heart's electrical impulses
If you are asked to complete an exam, when medically possible you should not:
