Prior to participating in the program, employees must sign a Release of Information Authorization Form.
Completed signed forms must be emailed or faxed to New York Life Group Benefit Solutions before employees can be contacted by a vocational coach. The email and fax numbers are listed on the form.
The form allows the coach to communicate recommendations with the employer’s contact person while also at times connecting the employee with no-cost health, wellness, and vocational programs.
Once the referral form and authorization are received, the employee will be contacted within two business days to schedule a telephonic assessment appointment.
