Domestic violence confidentiality


New York Insurance Law § 2612 prohibits an insurer from discriminating against an individual who has been a victim of domestic violence. In addition, if you or another person within your care is a victim of domestic violence and are covered under an insurance policy or annuity contract and have obtained a valid order of protection against the policy or contract owner, you may contact us to provide an alternative address and/or telephone number to receive company communications.

Please submit to us the order of protection you have obtained against the policy or contract owner and complete one of the attached forms below.* Once your request has been processed, the policy or contract owner will not be allowed to access any contact information we may have about you including your alternative address and telephone number.

* If you are covered under a health insurance policy, an order of protection is not required. Please submit to us the health information form attached below.
